Shadowing Request Form

The shadowing program has been paused for all candidates until December 1, 2025. Fourth-year medical students and other candidates interested in shadowing will have their materials reviewed in preparation for applicant interviews.

To begin the process, the requesting shadower must first fill out and submit the form below and upload all completed documents listed. Once all files have been received, the coordinator will process them through the department and the College of Medicine.
